Resident Outcome Goals

At SentosaCare, our goal is to meet individual and specialized demand for the care and supervision of our residents. SentosaCare provides a full continuum of rehabilitative services for diseased or injured as well as specialized clinical subacute services.

The goal of SentosaCare is to be a leading provider of outcome focused rehabilitation services and for illness and injuries requiring specialized subacute services. SentosaCare strives to maintain a satisfaction rating of above 95% from residents, families and payer. To this end, we have chosen to integrate a Pathway methodology for care delivery.

The pathway approach is SentosaCare's concept and philosophy that is incorporated into each resident's care plan. The team approach enables each resident to follow a customized pathway to recovery, so they can maximize their functioning and remain as independent as possible for a successful return home.

Each Pathway is divided into five phases, phase one being day of admission and continuing on to phase five as reflective of the resident status at the time of discharge.

Each Pathway consists of specific individualized sections as follows:

  • Self-Care/ADL
  • Cognition
  • Behavioral
  • Psychosocial
  • Education
  • Discharge Planning
  • Community Re-entry

  • Cardiac Rehabilitation
  • Chronic Progressive Neurological disorders
  • Osteoporosis
  • Cerebral Vascular Accident
  • Total Hip Replacement
  • Respiratory Rehabilitation

All Pathways incorporate a variance reporting and tracking system designed to monitor standard deviations from expected outcomes. The interdisciplinary team meets daily for informal rounds to evaluate outcomes and progress. Formal meetings are held weekly to review variance reports and modify care plans.
